NYS DDPC Surveys
The New York State Developmental Disabilities Planning Council makes
extensive use of electronic surveys in order to more accurately
identify the issues confronting people with disabilities and their families.
By collecting survey information, the NYS DDPC is able to develop
demonstration programs which improve the delivery of services and
supports to people with disabilities and their families.
Below is an index of currently open surveys. Please read each survey
description carefully in order to determine which survey(s) apply to
you.
Consumer Satisfaction Survey
We’re always looking for ways to improve what we do.
If you have received services or supports within the last three
years through a program funded by the New York State Developmental
Disabilities Planning Council, we would like you to take our
Consumer Satisfaction Survey.
It's important that the NYS DDPC knows what impact NYS DDPC-funded
programs have had on your life. Your input will assist us in making
NYS DDPC-funded programs event better, and will help us to
further-improve the delivery of services and supports to individuals
with disabilities and their families. After completing the survey
you will be directed back to the NYS DDPC Website.
